CB provided vocal to Lumpy Gravy uncredited.
CB played sax for one of the Royce Hall Concerts that were the basis for Orchestral Favorites - at the very end of one of the concerts, according to Matt Groening. Matt reported that at the end of the concert, after all the musicians had left, Don appeared, sax in hand. He played freely for a few minutes.
Jeezus! Good luck piecing this one together! What a conveluted mess...

Grow Fins '65-'82 CD featuring FZ
This 5 CD set is pure Beefheart gold.
CDs 3 & 4 will give you an idea of the enormity of the task FZ took on in producing "Trout Mask Replica". Very amusing too. CD 5 - track 15 has a brilliant FZ acoustic with Don in great voice, sounding more like Howlin' Wolf than ever...
There's also a book bound into the sleeve that is jam-packed with info.
(My copy cost £85 eight years ago, so it might not be easy to find).

Don't know what context your question is in, but:
Captain Beefheart made an album titled The Spotlight Kid in 1971. Reprise Records K 44162.
FZ doesn't feature, but members of his band/s do.
